The Type D mode conditions are as follows:
- Maximum of 12 persons per table
- Operate at 100% capacity
- For all table bookings, at least two-thirds of persons at the table must have received at least the first dose of a COVID-19 vaccine. The host Member shall be responsible for ensuring that his / her guests fulfill the required vaccination requirement.
- All relevant persons must present the QR Code of their vaccination records for verification before seating. If an individual received the vaccination outside of Hong Kong, he / she must make a declaration using a specified form prescribed by the government.
- All Members and guests must scan the “LeaveHomeSafe” (“LHS”) QR Code for that particular outlet using the LHS mobile App. Persons aged 65 or above, persons with disability and other persons recognised by the Government or organisation(s) authorised by the Government may fill out the prescribed form (click here for the form) to provide the required particulars as an alternative to scanning of LHS.
